We are located right off of Interstate 43 in Southeast Mequon. Our address is 1001 W Glen Oaks Ln Suite 112 Mequon, WI 53092. As you enter the Glen Oaks Office Park, we are located in the first building on your left. Our office is located at the end of the hallway on the first floor. The suites are not numbered in order. The easiest way to enter the building is to take a left as soon as you enter the office park, drive to the south end of the building where you will see a gazebo. Enter the door on the south side of the building and we are the first suite on your right after you walk through the mailroom.
Wisconsin is a Direct Access state, meaning patients can see a physical therapist without needing a healthcare provider's prescription. However, some insurance plans require a prescription to cover services. If you plan to submit your claim to your insurance company, check with your plan if you need a referral for reimbursement.
Cultivate Your Wellbeing is a fee-for-service provider and does not participate in any insurance plans. That allows more time for individualized treatment, and allows the session to cater to what the patient needs, not what insurance will cover. While most of our patients do have health insurance, they choose to forgo it in order to receive expert physical therapy care and often end up paying less once high deductibles and copays are taken into consideration. Every patient will be provided a bill at the end of their session with all information needed to file a claim with your insurance for reimbursement per your plan.

If you are a current patient you can also schedule your sessions through your PtEverywhere App. If you are unable to find a time that works for you make sure to use the "waitlist feature" on our scheduling page to automatically be notified of openings and/or email us at feelbetter@cultivateyourwellbeing.com and we will do our best to get you in!
Make sure to fill out all intake forms prior to your first visit so your therapist can review them ahead of time and familiarize themselves with your history. This maximizes the amount of time spent diving into your specific concerns and providing treatment during your first session. Many patients are nervous and do not know what to expect for their first pelvic floor therapy session. Please take a breath and relax. Care is provided in a supportive environment and all questions will be answered leaving you in complete control of what your treatment entails. In general, we recommend wearing clothing that is easy to move around in and portions of the exam may require undressing and draping with a sheet or gown.
This varies for each patient but in general we start with sessions one time per week then decrease frequency as you progress to every two, three, or four weeks depending on your progress and goals.

As a courtesy we ask for as much notice as possible when you know you need to cancel an appointment to allow another patient to schedule. We require a 24 hour notice of cancellation. Last minute cancellations or not showing up to a scheduled appointment without notice is subject to our $75 cancellation fee.
Cultivate Your Wellbeing is not a Medicare Provider so the law prevents us from providing Medicare patients with what would be considered “normally covered services.” If you are a Medicare beneficiary and would like to request treatment at our clinic, please read below and call us at (262) 417-1241.
Medicare Beneficiaries: The US government has laws that control where Medicare beneficiaries can spend their healthcare dollar and persuade healthcare providers to enroll in their system. Because we are not Participating Medicare Providers, we can only accept Medicare beneficiaries as patients when the patient does not want Medicare billed for any PT services. This request to not involve Medicare in payment must be made up front by the patient and be made of the patient’s own free will. In other words, if you’re a Medicare beneficiary and are adamant about seeing us for your care even though we are not participating Medicare providers, we can help. However, the only way we can provide you with PT services is when you truly don’t want Medicare involved and you ask up front that Medicare not be billed or involved in your physical therapy care. If you do want to use your Medicare benefits for physical therapy, we cannot provide you with treatment at our clinic but we can help you find a good Medicare provider in your area.
In order to have a video chat with your therapist you must be logged into your PtEverywhere account. Your therapist will call you through your account at your scheduled appointment time.
